Step-by-Step ULV Testing
- Assess R wave – must be >5mV (in some cases >7mV)
- Record 12 lead ECG at 100mm/s sweep speed
- Deliver pace train from RVA at 500ms cycle length
- Measure time to latest peaking monophasic T wave
- Select shock strength (start 20J)
- Scan at 0, -20ms, +20ms (rare to go out to + 40ms, but has been done)
- Wait 60s between shocks
- If scan does not induce VF – you are done!
- If you want to determine ULV, decrease shock strength
- Repeat until induce VF
- If induced, previous tested is ULV
- If induced at 20J, increase shock strength (high DFT)
