Step-by-Step ULV Testing

  1. Assess R wave – must be >5mV (in some cases >7mV)
  2. Record 12 lead ECG at 100mm/s sweep speed
  3. Deliver pace train from RVA at 500ms cycle length
  4. Measure time to latest peaking monophasic T wave
  5. Select shock strength (start 20J)
  6. Scan at 0, -20ms, +20ms (rare to go out to + 40ms, but has been done)
  7. Wait 60s between shocks
  8. If scan does not induce VF – you are done!
  9. If you want to determine ULV, decrease shock strength
  10. Repeat until induce VF
  11. If induced, previous tested is ULV
  12. If induced at 20J, increase shock strength (high DFT)
