Indications for Pacing in Setting of Acute MI

This system is a point system based off of the ACC guidelines

  • Step 1:Check the AV Conduction
    • No conduction problems = 0 points
    • First Degree AV Block = 1 point
    • Second Degree Type 1 = 1.5 points
    • Second Degree Type 2 = 2 points
  • Step 2:Check the Intraventricular Conduction
    • No conduction problems = 0 points
    • Fascicular block = 1 point
    • Old Bundle Branch Block = 1 point
    • New Bundle Branch Block = 2 points
    • Fascicular and Right Bundle Branch Block = 2 points
    • Alternating Right and Left Bundle = 2 points
  • Add up points
    • Observe for 0-1 points
    • Transcutaneous Pacing for 2-3 points
    • Transvenous Pacing for 4 points
