Interpretation of Exercise Stress Test

ST Segments Normal Except

  • Borderline 0.5 – 0.9mm
  • ST Elevation in areas of prior MI
  • T wave inversion
  • Pseudonormalization

ST segments Abnormal

  • 1 mm Horizontal or Downsloping ST depression
  • 1.5 mm upsloping ST depression
  • 1 mm Upsloping ST with angina
  • >1 mm ST elevation

ST segments Very Abnormal

  • >2 mm Horizontal or Downsloping ST depression
  • >2.5 mm Upsloping ST depression
  • >2 mm ST elevation

Termination of Stress Test

  • Drop in BP >10
  • Moderate to Severe Angina
  • ST Elevation > 1 mm
  • ST Depression >2 horizontal or downsloping
  • Excessive BP >250 systolic and > 155 diastolic
  • Arrhythmia

Features of High Risk Stress Test

  • >2mm ST depression
  • >1mm at low level of exercise
  • ST depression in greater than 5 leads
  • ST elevation
  • Hypotension
  • Ventricular Tachycardia
  • ST changes greater than 5 minutes into recovery
